Athletic Physical Forms
All High School and incoming Freshman Athletes for
any LISD High School must have a Physical, Paperwork, and Concussion acknowledgement on file with the Athletic Trainer before you are able to participate in any athletic practices.
Summer Assignments for Students
Students who have signed up for any Pre- AP, Pre- IB, AP or IB course for Fall 2012 should check the link below for the summer assignments required for these courses. All summer reading or work should be completed by the first day of school. This work does receive a grade.

Voluntary transfers will be permitted to transfer to other open high school campuses within the district on a space and staffing available basis.
